The character of Dr. Colin MacIver first appeared on the One Life to Live canvas in May 2000. Introduced as a mysterious hero, Colin saved Nora Buchanan from the train which crashed while taking Will Rappaport to Statesville Prison. Colin cared for Nora at the Cherryvale clinic, affectionately nicknaming his patient "Scarlet" while she was unable to speak.
"Someone's missing you, aren't they? There's gotta be a whole lotta people out there looking for a woman as beautiful as you."
While at the hospital, the charming doc who has a way with the nurses, was reunited with his wife. Dr. Melanie MacIver, Bo Buchanan's one night stand earlier that month, informed her cheating husband that she planned to end their marriage.

Despite his troubled personal life, Colin was Viki's doctor while she was treated for breast cancer and he continued to care for Nora. She spoke Lindsay Rappaport's name and Colin phoned his sister-in-law. Lindsay was shocked to see that her nemesis was alive and begged Colin to keep her dead, just for a while. He reluctantly agreed and made a deal with Lindsay; if he kept Nora hidden away for a few weeks, Lindsay would help him convince Melanie not to leave him.
Colin kept Nora in his home, all the while caring for her. She began to catch on and realized her doctor was not just treating her injuries, but keeping her away from home. Colin told Nora he was making her well and wanted to see her reuinted with her family, but something was in the way.
"There must be two Colins. There has to be. There's the Colin that's holding me captive, then there's this Colin -- the one who's healing me."
Nora's son, Matthew, fell ill. Colin treated him at the Llanview Hospital and after Nora overheard a phone conversation, he had to let her know that her son was sick. While Nora wept over not being able to be there for him, Colin's nephew, Will Rappaport, snuck into the house. He heard a woman crying but Colin managed to convince him it was just his emotional, loud girlfriend.
Eventually Nora and Will began to communicate through the vent in the bedroom leading to the basement. He was about to help her escape Colin's house when Todd Manning convinced him everything was taken care of and he should leave. Will had knocked Colin unconcious in the basement and he was unaware that Lindsay came into the house and up to the bedroom.

Lindsay feigned shock at seeing Nora alive. But soon the two women taunted each other and Nora began to call Colin for help. He struggled up the basement stairs but he was too late; Lindsay injected Nora with a mind erasing drug. Colin rushed to the hospital for the antidote and gave it to Nora.
"You're right, I am guilty. I'm materialistic, I'm self-absorbed and I'm manipulative. I'm everything Lanie's accused me of. But I tried to help that woman and it was long before I ever knew who she was."
Before he could know how the anitdote affected her, Nora disappeared from Colin's house. He hired private investigators to look for her and dodged Sam and Bo's accusations that he ever knew Nora. Once Todd Manning brought Nora back to Llanview, a relieved Colin began to befriend her.

Will got word to Sam that he saw Nora in Colin's house. Colin was arrested, but he had ammunition in the form of the tape with Asa Buchanan's confession; the very tape Will needed to be free. Colin struck a deal and was set free. He continued to blackmail Lindsay for money, and if she didn't pay up, he would reveal her role in what happened to Nora.
After confessing to keeping Nora in his home and caring for her after the wreck, Melanie continued with her plans to divorce Colin. She also worked to strip him of his medical license. Although he confessed, Colin continued to plead with Nora that he never hurt her and wanted her to see that they shared a deep connection.
"I want you to get to know me, but you can't do that if you're still afraid."
Colin continued to blackmail Lindsay, pursue Nora and wreak havoc in everyone's lives, including Roseanne Delgado and Lanie. He threatened to spill his ex-wife's secrets in regard to the mercy killing of her father if she did not pay him. But despite his bad behavior, he was able to spend time with Nora - it was all part of her plan to get Colin to name Lindsay as her kidnapper.
Nora pretended to have romantic feelings for Colin, hoping that would inspire him to reveal the entire truth to her. She was unaware that his affection for her had become an obsession and that he had built a shrine to her in his bedroom.

To protect Nora, Asa enlisted Max as her bodyguard. He was always nearby when Nora was alone with Colin, except for one night when Colin lured Nora to his house by saying he was going to leave town. Once up in the bedroom, Nora couldn't help but become frustrated with Colin's ultimatum - he would tell her the truth if she slept with him. He was devestated and outraged when he caught onto Nora's plan. After Asa rescued Nora, he destroyed the shrine.
His destruction wasn't the end of his obsession. It was only the beginning. Colin's plans for Nora culimnated in one destructive night that included a dinner party he organized. He gathered all of his enemies in one room and spilled secrets of the Rappaport family, as well as announced he was leaving town.
"Goodbye! I am leaving this lousy town. That's right. I should've down it sooner. The first true love of my life left me, and she took my livelihood with her. Thank you very much! And the second true love of my life betrayed me. She played with my heart."
After the dinner party, Colin lurked about Nora's house. When she was alone, he injected her with a drug and took her to his house. He intended to take Nora out of the country, but his plans were put to an end by a knock on the door.
The rest of the night remained a mystery for many weeks. That was Colin's last night, and after a series of visitors and confrontations, he attempted to return to Nora in the bedroom. She stopped him, and Colin fell down the stairs to his death. It was only weeks after his death that Nora remembered being the one to push him.

His death was not the last anyone in Llanview would see of him. Colin appeared via videotape at the reading of his will and left everyone with strange mementos of their time with him. Perhaps the biggest reminder of his life and death, though, is the arrival of his twin brother, Troy.
